Rehabilitation Case Management

For more complex injury presentations, such as serious, complex and catastrophic injuries, Overland Health work alongside specialist Rehabilitation Case Managers to work with individuals to assess and manage their needs, coordinate their care and overcome barriers to recovery and regain independence, function and return-to-work.

Case Management is a collaborative process which: assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual’s health, social care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes.

Following the Rehabilitation Code (2015) and on a joint instruction funding basis, we work with strategic partner organisations to provide nationwide Case Management services for moderate to complex and catastrophic injuries. Our services include telephone and field based initial needs assessments, combined assessments, rehabilitation and injury case management services.
